Privacy Policy
This privacy statement describes how I, Polly Hockaday, protect and make use of the information you give me. If you provide or are asked to provide information when contacting me, it will only be used in the ways described in this privacy statement.
Polly Hockaday Counselling needs to gather and use certain information to enable you to work with me safely and effectively, and to enable an efficient system for appointment reminder and invoicing.
The information I gather
Name and address,
Contact telephone number and email address
Date of birth
GP details
Any other information which might inform the way we work together ie medication, medical diagnosis
During the course of initial contact and then subsequent therapy, I will inevitably also collect a significant amount of other personal data relevant to assessing and working with your presenting issues ie to inform and support the service you have sought from me.
How I use this Data
1. To communicate with you when necessary
2. To arrange/rearrange appointments
3 To invoice when necessary
4. To contact your GP if necessary and with your permission.
Controlling information about you
Any personal information I hold about you is stored in line with the Data Protection Act 1998 and the General Data Protection Regulation 2018. Your data will be kept for the lifetime of your status as a client with me and then for one year, unless there are reasonable grounds for keeping it for a longer period of 3 year.
Security
I will always hold your information securely. All client files are therapy notes are kept secure in a locked filing cabinet. My client work is shared only with my Clinical Supervisor.
Data Accuracy
If during the course of your contact with me any personal data changes ie you move house, change your GP or change your name. I would be grateful if you would keep me fully informed so I can ensure my records are up to date.
Subject access requests.
All individuals who are subject of personal data held by Polly Hockaday Counselling are entitled to:
Ask what information is held by me and why.
Ask how to gain access to it.
If you would like to request a copy of the data I hold about you, this is called a subject access request. Subject access requests should be discussed and then requested in writing. I will aim to provide the relevant data within one month.
I will only release the data to the client (yourself) in the first instance, on completion of the relevant paperwork.
